As an employee who was eligible for minimum essential health coverage through the GoJet Airlines Health and Welfare Plan, you are entitled to receive Form 1095-C for the 2025 tax year. This form serves as proof of the health coverage that was offered to you.

Form 1095-C was previously mailed to eligible individuals. However, due to updated regulations intended to reduce paperwork and administrative burden, we are no longer required to automatically mail paper copies.

For most employees and in most states, Form 1095-C is not required to be filed with your personal tax return. If you would like a free copy of your Form 1095-C for your records, you may request one using any of the methods below:

A paper copy will be mailed to the address on file within 30 days of receiving your request.

Electronic Copy Option

If you prefer to receive your Form 1095-C electronically, please include your preferred email address in your request and specify that you would like a PDF copy. You may also access your Form 1095-C electronically through ADP Workforce Now at: www.workforcenow.adp.com
